Needed Information on the MTLs

Every Multiple Transaction Log (MTL) must have the following information recorded:

• Time
  

• Patron’s Name
  

• Patron’s permanent address
  

• Type and number of documents used to verify the patron’s name and permanent address
  

• Date of birth
  

• Type of transaction
  

• Dollar amount
  

• Employee signature and license badge number
  

Once you have read this list, please go back and read through the list again.  It is imperative that you record each of aforementioned on MTLs in case the customer exceeds the threshold and is not available to provide the information later.
